APPOINTMENTS

We realize how important your time is to you. Due to the nature of our practice there are times when unforeseen emergencies occur during scheduled appointment hours. Every attempt will be made by our office staff to contact you in these cases to either reschedule an appointment if your doctor has another type of emergency. Please inform us if your home telephone number, work number, or cell phone number has changed as well as your mailing address. We also ask that you kindly give us 24 hours of notice if you need to cancel or reschedule an appointment, so that we may make this time available for another patient. 



  • There will be a $75 charge for new patients and $35 for established patients if you fail to call within 24 hours to cancel your routine appointment.  A $100 charge will be applied to your account for a missing an office procedure.  A $250 charge will be applied to your account for failing to cancel your surgery within 24 hours.


  • *Effective Immediately!  Due to an increase in noncompliance:  No Show fees will be doubled for missed appointments Wednesday evening and Saturdays.


  • Coinsurance will be due before any procedure is performed.  Failure to comply will result in cancellation of the procedure.



NEW PATIENT IDENTIFICATION

The regulations require each business to establish a customer/identification process appropriate for the size and type of business. In our office we will obtain and record the following information of new patients. Please have the following information available when you come for your appointment:

Name

Address

Date of Birth
Social Security Number/TIN

Driver’s License

Other Picture ID

TREATMENT OF MINORS

A parent or legal guardian must accompany a minor to their first appointment and provide ID. Parents should make arrangements for payment at this and any subsequent visit (due at the time of visit). All contraceptive and venereal disease visits are protected per Virginia code.

OFFICE HOURS


Office hours are Monday, Tuesday, and Thursday 9:00am to 5:00pm, Wednesday 11:30 to 6:30, and Friday 9:00am t0 1:00pm. Due to inclement weather these hours could vary. Every effort is made to notify our scheduled patients of any cancellations due to weather.


TELEPHONE CALLS

Please make an appointment for urgent problems as general medical questions may not be returned for 2-3 business days.

MEDICAL EMERGENCIES

In the event of a medical emergency after hours or on weekends, please call Winchester Medical Center (540) 536-8000 and have the operator page the physician on call for our practice.

FEE FOR SERVICES

Payment is expected at the time services are rendered. The co-pay is due at the time of service and will be collected by the receptionist when you check in. Your insurance will be billed for the balance as a courtesy for you. If you do not have insurance, fees for that visit are due at the time of the visit unless other prior arrangements have been made. Any pathology fee that you might incur will be billed to you directly by the pathologist and laboratory fees will be billed through Sunrise or Quest unless you notify us of a different lab that your plan participates with. It is the responsibility of the patient to know what lab your insurance company participates with. The bill is still the responsibility of the patient regardless of insurance. We honor Visa, MasterCard, Discover, American Express, and Care Credit.
